WHAT IS CASE MANAGEMENT?
Case Management is the process by which a professional, either a nurse or social worker, assists the beneficiary in meeting his or her specific needs.  The case manager works with the beneficiary to identify those needs, and the necessary steps to meet them. Case management also allows the beneficiary to have someone on his or her side when it comes to making decisions that affect the individual's health and well-being.

WHO IS ELIGIBLE FOR CASE MANAGEMENT?
Case Management services are available to beneficiaries who are eligible for treatment at McConnell Medical Clinic and meet certain criteria. Those individuals most likely to receive Case Management are beneficiaries who have a high-risk, catastrophic, or extraordinary condition like cancer, major burns, organ transplant, high-risk pregnancy, PTSD, traumatic brain injury, or multiple complex medical diagnoses. This is by no means a comprehensive list. If you do not suffer from these conditions, it does not mean you do not qualify for Case Management services.  Each beneficiary referred to Case Management either by self-referral, Primary Care Manager, or other means, receives a complete evaluation to determine eligibility.

WHAT IS REQUIRED FOR CASE MANAGEMENT?
Anyone who receives Case Management services is required to fill out a form for the release of your information. The link below will open a copy of the form that you may print, fill out, and bring to your initial Case Management appointment.
